This is the first segment of The Meadows’ staff nurse, Judy Beard R.N., giving deposition testimony (2004) regarding patient assessment and policy compliance in reference to THE MEADOWS of Wickenburg’s Nursing Policy #750.

THE MEADOWS of Wickenburg is the renowned, remote and private facility that has treated such glitterati and public figures as Rush Limbaugh, Kate Moss, Elle MacPherson, Cindy McCain, and Eric Benet. THE MEADOWS is licensed as a Level 1 Psychiatric Acute Hospital by the State of AZ, Division of Behavioral Health Licensing and has full JCAHO accreditation. Marketed as a “dual diagnosis” facility, THE MEADOWS claims to treat addictions (e.g. drug, alcohol, sexual, work, etc.) as well as affective disorders such as anxiety, severe depression, PTSD, and Bi-Polar disorder. In spite of these facts, THE MEADOWS operates as an open campus with no secure or locked units. Furthermore, it has been grand-fathered out of a second AZ Department of Health License, a Hospital Facilities License, that would require THE MEADOWS to comply with Arizona hospital standards, regulations, and requirements.

On November 28, 2004, MEADOWS’ inpatient, R. Matthew “Matt” Burne, was able to walk out of the Acute Wing (Kiva) of this Level 1 Psychiatric Acute Hospital sometime during the middle of the night – through one of the unlocked and unmonitored doors – unnoticed and unobserved by any staff. Matt then hanged himself with his own belt (that was never confiscated by MEADOWS’ staff) from a tree on a horse farm adjacent to THE MEADOWS property. He had been at THE MEADOWS for only four days, during which time staff repeatedly documented Matt’s high levels of anxiety, depression and verbalized thoughts of hanging himself.

Per THE MEADOWS’ own internal nursing policy, #NSG750, inpatient Matt Burne should have been on Eye-to-Eye observation. Instead, and in spite of his anxiety, depression, sucidality, as well as Policy #NSG750, Matt was was placed on Q4 hours observation by his MEADOWS’ attending psychiatrist, Dr. Stephen Swift Brockway. This required staff to check on him only once every four hours and allowed Matt the opportunity to leave the Acute Wing (undetected) and complete suicide off premises.

Nurse Judy Beard was the last nurse to complete a suicide assessment on Matt Burne. She did so at bedtime on the evening before he completed suicide.
